Rotation Paperwork Documentation – IMPORTANT for Rotations!

Every student has access to the Student Clinical Manual in Student Resources under the tab for your course level. Be SURE you read this carefully before going to rotations. Also, here are some clarifications on MANDATORY requirements for rotations and their documentation:

1. You MUST take your tablet PC or other mobile device ready for your documentation with you. You cannot accurately and honestly document after the rotation is over, and you most certainly cannot obtain a preceptor’s signature verifying you were there without it. The mobile app has an offline feature so consider downloading and executing that app and using it per the post before this one in these Announcements.

2. You MUST put in the CORRECT first name, last name and license/certification of your preceptor as well as the person’s email address and any other information it requests IF the preceptor is not already listed in the drop down menu. So first, look to see if they exist in the system as a preceptor choice to avoid adding the same preceptor over and over again in the system. If not, open the page for setup, hand the device to the preceptor, and ask him/her to enter the requested information to set up a preceptor account so he/she can see you data and complete the evaluation on your performance for the shift. Explain to the preceptor that it is IMPERATIVE that they go in and finish the preceptor evaluation or you WILL LOSE CREDIT FOR THAT SHIFT, ITS HOURS, ITS CONTACTS AND YOUR SKILLS! We cannot take you at your word – everything you enter has to be verified and rated by your preceptor.

3. When you finish entering your data for the shift and hit “Submit”, the very next item that should appear is a signature box for your preceptor to sign verifying you were present and in that rotation. The preceptor needs to sign here. Once you click to accept, the form will move on to allow you to submit your data for preceptor evaluation to be emailed to the preceptor and for clinical faculty grading.

4. REMEMBER, IF YOU DO NOT OBTAIN THE PRECEPTOR EVALUATION OF YOUR DATA SUBMISSIONS FOR YOUR SHIFT AND YOUR PERFORMANCE AND BEHAVIOR BEFORE YOU LEAVE THE SITE, YOU WILL LOSE CREDIT FOR THE SHIFT. THIS IS YOUR RESPONSIBILITY!
